Land Mesh Access SSO in git; fix Connect grant order at FileVault lock screen.

- Mesh Access SSO stack — mesh_access_grants, meshAccessIdentity, session Terminal + Copy files, MCP connect_screen / connect_ssh / file copy (landed in git)
Fixed
- Connect at lock screen — Screen no longer bails on filevaultLocked before requesting a grant; target Rig can run unlock-on-grant when Connect-ready is enabled
- Grant-first Screen — openScreenWithGrant is the committed path (wake → grant → unlock → VNC credential)
